I'm a Reform rabbi in San Antonio, Texas. I have officiated many LGBTQ+ weddings and conversions. I am always open for conversations and requests for discussions.

I put currently learning for many of the answers because I have not had the opportunity to experience some of these things, but am aware and have plans for how to do them (i.e. LGBTQ+ end of life care, I have not had specific experience around taharah for the LGBTQ+ community, but am in constant communication with our Chevra Kaddisha).
